Case Management

Mental health symptoms can sometimes make everyday responsibilities difficult.

MVGC case management services provide practical, individualized support to help clients improve stability, increase independence, strengthen recovery skills, and remain successfully involved in their communities.

A case manager works with the client to identify needs and develop goals based on what is most important to that individual.

Case management may address independent living skills, community resources, health care coordination, housing stability, employment resources, education, transportation, benefits, appointment management, treatment follow-through, coping skills, symptom management, social skills, problem-solving, decision-making, and coordination among service providers.

Case managers may also help clients practice skills in real-life settings so that they can become increasingly independent.

Recovery-Focused

Case management is not about doing everything for the individual.

The goal is to work with the individual to strengthen skills, identify resources, overcome barriers, and increase independence.
